Care Coordinators @ TOUCH Caregivers Support (Toa Payoh/ Jurong)

Responsibilities:

• Support and journey with caregivers of the frail elderly
• Assess the needs of the frail elderly and his/her support network
• Make recommendations and referrals for appropriate health and social services
• Help caregivers negotiate health and social services
• Assist in relocation of frail elderly to other living arrangements

Requirements:

• Occupational Therapist / Social Worker / State Registered Nurse with minimum 3 years experience in geriatric care preferred
• Familiar with the eldercare landscape (health and social services) of Singapore
• Able to converse in Mandarin and / or dialects will be an added advantage
• Relate well with elderly and caregivers

This full-time position is only available to Singaporeans and Singapore Permanent Residents.
