Senior / Medical Social Worker @ TOUCH Home Care (Toa Payoh/ Jurong)

Responsibilities

• Provide direct social work services for the frail home-bound elderly seniors and their family members including assessment, intervention and counselling
• Be part of a multi-disciplinary team and network with external organisations to provide holistic and seamless care
• For Senior Position, provide clinical supervision for support staff and interns involved in casework, case management of elderly with complex social and health cases, and initiate new programmes to enhance quality of life for the elderly and their caregivers

Requirements

• Master/Degree/Graduate Diploma in Social Work, registered with SASW
• At least 2 years experience in geriatric care or social work
• Able to converse in with elderly in Mandarin and dialects
• For Senior Position, at least 3 years of relevant supervisory experience

This full-time position is only available to Singaporeans and Singapore Permanent Residents.
